75+ Health Assessment (MBS 703)
If you are 75 years or older, Medicare funds an annual comprehensive health check called the 75+ Health Assessment (MBS item 703). It is one of the most under-used Medicare benefits in Australia — and one of the most useful. At MediPharm Hoxton Park, this assessment is bulk-billed for all patients with a valid Medicare card holding a valid Medicare card, and includes:
- Comprehensive medical, physical, psychological and social review
- Medication reconciliation — including over-the-counter and supplements
- Falls and mobility assessment
- Continence, vision, hearing and dental review
- Cognitive screening (memory) and mood
- Home safety, social supports and carer arrangements
- A written plan you can take home and share with family or carers
Chronic Disease Management — GPMP & TCA
If you have two or more chronic conditions — for example diabetes, COPD, heart disease, arthritis, kidney disease or osteoporosis — you are eligible for a Medicare-funded GP Management Plan (GPMP, item 721) and Team Care Arrangement (TCA, item 723).
This means we sit down with you, map out your conditions, set goals, and arrange up to 5 bulk-billed allied-health visits per calendar year — podiatry, physiotherapy, dietitian, diabetes educator, exercise physiologist. The plan is reviewed every 3 months.
Dr Abbas Al-Attar and Dr Ebtesam Ford both write care plans at MediPharm. Allied health is on-site (physiotherapy, podiatry, dietitian) so the team can communicate directly.

Is the 75+ Health Assessment free?
For eligible patients with a valid Medicare card, MediPharm bulk-bills the 75+ Health Assessment (MBS 703), so there is no out-of-pocket cost. The assessment takes around 45–60 minutes and is conducted once every 12 months.
Do you speak Arabic, Persian or Farsi?
Yes. Dr Abbas Al-Attar, Dr Sam Zahrouni and Dr Ebtesam Ford all speak Arabic. Dr Sam Zahrouni also speaks Persian (Farsi). The reception team is multilingual and 13+ languages are spoken on-site.
How do I get a GP Management Plan for my elderly parent?
Book a longer appointment with Dr Abbas Al-Attar or Dr Ebtesam Ford and let reception know it is for a chronic-disease care plan. The first plan visit takes about 45 minutes and is bulk-billed for all patients with a valid Medicare card. We will write the plan, arrange up to 5 bulk-billed allied-health visits across the year, and review the plan every 3 months.
Is shingles vaccine free for over 65s?
Yes. Under the National Immunisation Program, the shingles vaccine (Shingrix) is free for non-Indigenous Australians aged 65 and over, and free for Aboriginal & Torres Strait Islander people aged 50 and over.
Can a family member or carer attend the appointment?
Absolutely — we encourage it. Bringing a family member or carer helps with history-taking, medication review and remembering instructions. Let reception know at booking and we can arrange a longer consultation slot.
